Job Description:
A company is looking for a Registered Nurse Care Coach to work
remotely and support patients in a Chronic Care Management
Program.
Key Responsibilities:
Utilize care management software to conduct monthly calls with
Medicare patients with chronic conditions
Build rapport and coach patients on self-management strategies and
health improvement
Implement and update the Plan of Care, connecting patients with
necessary community resources
Required Qualifications:
Current, unrestricted Compact License
3+ years of experience as a Registered Nurse
Proficiency with electronic health records and web-based
applications
Self-directed and able to work independently with minimal
supervision
Good technological skills and eagerness to learn new software
